It is fraudulent to misrepresent yourself as another person to gain access to Protected Health Information.
We provide two levels of access to plan information on this site. Level 1 access is for both members and covered dependents and provides access to general information about your coverage and plan. Level 2 access is limited to members only and will provide access to specific information about your claims.
Level 1 – General Information – for members and covered dependents
If you have coverage through Ameritas under a group plan, you are authorized to access general information about your plan here, including your certificate of coverage and dental benefit summary.
To access this information, you’ll need to create a User ID and password, then provide the member’s name, date of birth and ZIP Code.
Level 2 – Protected Health Information – for members only
With a Level 2 account you can access specific health information about yourself and your covered dependents. Only the member is authorized to create a Level 2 account. The member is defined as the primary insured person who has completed eligibility requirements and elected to enroll in the plan. Eligible dependents enrolled in the member’s plan are considered covered dependents.
As the member, you can view pending and paid claims, pretreatment estimates and dental or orthodontic maximums and deductibles for yourself and your covered dependents.
