Individuals and families
Individual dental and vision insurance is for those who don’t have coverage through an employer or elsewhere. Ameritas dental insurance plans have no waiting periods in most states. There are no enrollment fees on any plan.
Dental
Ameritas PrimeStar® dental plans range from basic benefits to robust coverage. Plan benefit options include increasing coverage levels and annual maximums. Some plans include additional benefits, like orthodontia.
Vision
All PrimeStar vision plans offer next-day coverage and no waiting periods. You can choose from plans that feature the EyeMed or VSP vision network. Some states offer a no-network vision plan.

Individual dental and vision FAQs
How long is the contract?
There is no annual contract. You pay monthly, with no change in premium rates guaranteed for 12 months, and can cancel anytime.
Do I need to visit a network provider?
No. You can visit any dental or vision provider, in- or out-of-network. Family members do not need to visit the same provider. Visiting a network provider can make benefit dollars go further.
Is there a waiting period before benefits begin?
PrimeStar dental plans do not include waiting periods in most states. If there is a waiting period, it will be applied to a specific list of procedures. Some plan benefits increase after the first year to maximize your policy value.
When is the plan effective?
Coverage can begin as soon as the day after you enroll. Or you can choose an effective date up to 90 days in the future.
How do I submit a claim?
When visiting a network dental or vision provider, there are no claim forms to submit. To submit an out-of-network claim, access the claim form on the Forms/Disclosures page under the Health menu.
Is there a minimum age to enroll?
To enroll in a policy, you must be at least 18 years old. Those under 18 can be added as dependents under their parent’s plan.
